Personnel Accident Insurance is an affordable supplement to life insurance.
This plan is optional and covers accidental death, loss of one or more limb(s), loss of one or both eye(s) or Permanent Total Disablement.
It is written through Lloyd's of London and purchased by the employer for Full-time employees.
This coverage is 24 hour/ 365 days a year. Premium as low as $100.
There are three (3) plans to choose from:
Plan A
Accidental Death $75,000
Loss of one or more limb(s) $75,000
Loss of one or both eye(s) $75,000
Permanent Total Disablement $75,000
Plan B
Accidental Death $150,000
Loss of one or more limb(s) $150,000
Loss of one or both eye(s) $150,000
Permanent Total Disablement $150,000
Plan C
Accidental Death $250,000
Loss of one or more limb(s) $250,000
Loss of one or both eye(s) $250,000
Permanent Total Disablement $250,000
Eligibility:
All Full-time employees are eligible for coverage.
Extensions:
- Assault: We will pay for any claim caused or contributed by an unprovoked malicious assault by another person(s).
- Disappearance: If during the Period of Insurance and within the Territorial Limits, You disappear and after twelve months it is reasonable to believe You have died as a direct result of an Accident then We will pay the death benefit under the Policy Schedule subject to a signed undertaking that if the belief is subsequently found to be wrong, the benefit paid will be refunded to Us.
- Exposure: If during the Period of Insurance and within the Territorial Limits as a result of an Accident You sustain Bodily Injury, require Hospitalization, incur Medical Expenses or Die as a direct result of unavoidable exposure to the elements, We will compensate You up to the amount stated in the Policy Schedule.
- Funeral Costs: In addition to the Benefits We will pay up to a maximum of $5,000 for the funeral costs for Your burial or cremation following Your death due to an Accident during the Period of Insurance and within the Territorial Limits
